Video and Business Records Certification for Retail Investigations

Retail investigations often rely on surveillance footage, video, audio and other business records to document incidents and support legal proceedings. This certification form provides retail asset protection and investigative teams with a standardized way to authenticate recovered multimedia and other records and establish that the materials are true and correct copies.

The form allows a custodian of records or other authorized person to certify that the records were created at or near the time of the event, based on information from someone with knowledge, and maintained as part of regularly conducted business activity. It also includes fields to identify individuals seen or heard in recovered multimedia and describe the records, video, audio or other data associated with the certification.
